Instructions for filling out the participation form:

  1. Select the participation type and number of schools participating in C-CAP for the 2018-19 school year. Please note that the “LAUSD Perkins Eligible” participant type is only for LAUSD schools receiving Perkins Funding for the 2018-19 school year.
  2. School/District Contact Information – input your school or district’s billing/invoice contact information. The email address entered here will receive the form confirmation email.
  3. Invoice Preference – Select the date you’d like to be invoiced.
  4. Please enter the School Principal’s contact info for each participating school in the contact info fields labeled with the participation type you initially selected (the header will read either “Participating School- Single Teacher,” “Participating School- Multiple Teachers,” or “LAUSD Perkins Eligible”). Make sure that your entries correspond with the appropriate participation type, which is noted in the label above the fields.
  5. Participating Schools- In the same order as the School Principal(s), enter the name of each participating school.
  6. Click “Register.”


Terms of Payment:

All invoices must be paid within 90 days. Payments made after 90 days will incur a 5% administrative late fee. If payment is not made within 90 days, C-CAP will reissue the invoice with the 5% late fee and schools will be asked to submit a PO # and remit payment within 30 days. No refunds will be given after services have been provided.
